        Here is this month's Parent Questionnaire.  The purpose of this communication is to help keep our lines of communication open.  Please fill out (only if necessary) and return this as soon as possible.  I'll do my best to contact you as soon as I can.  Sometimes I have a lot of requests to call parents, so please understand if I do not contact you for a few days.  I hope this proves to be a good form of communication between school and home this year.
